Mục tiêu: Nghiên cứu đánh giá hiệu quả thực phẩm ăn liền, bổ sung hàng ngày (RUSF (HebiMam) tăng cường 21 vi chất dinh dưỡng và cung cấp 225kcal/ngàylên tình trạng hemoglobin của phụ nữ mang thai ở 10 xã, huyện An Lão, thành phố Hải Phòng.

Đối tượng và phương pháp: 398 phụ nữ mang thai với tuổi thai từ 6 đến 16 tuần tham gia thử nghiệm ngẫu nhiên, trong đó phụ nữ mang thai nhận được ít nhất 20 tuần hoặc là sắt (58mg) và acid folic (400µg), hoặc bổ sung 15 vi chất dinh dưỡng bao gồm 30 mg sắt và 400µg acid folic hoặc Hebi-Mam bao gồm 30 mg sắt và 400µg acid folic. Hemoglobin được đo tại điều tra ban đầu (6-16 tuần thai) và kết thúc nghiên cứu (36 tuần thai) bằng phương pháp cyanmethemoglobin.

Kết quả: Sản phẩm nghiên cứu đã có hiệu quả trong việc cải thiện nồng độ hemoglobin tại thời điểm kết thúc nghiên cứu, với mức tăng trung bình 3,3g/l ở nhóm sắt acid folic, 2,7g/l ở nhóm bổ sung đa vi chất dinh dưỡng và 3,1g/lở nhóm sử dụng thực phẩm bổ sung (p <0,01). Tỷ lệ thiếu máu là 26,1%, 24,8% và 24,5% tương ứng. Một số yếu tố ảnh hưởng có ý nghĩa thống kê đến sự cải thiện nồng độ hemoglobin tại thời điểm kết thúc là nồng độ Hb trước can thiệp (p<0,001), dự trữ sắt cạn kiệt (p<0,01), tuần thai khi bắt đầu tham gia nghiên cứu (p<0,001) có mối tương quan nghịch với nồng độ hemoglobin kết thúc nghiên cứu.

Kết luận và khuyến nghị: RUSF Hebi-Mam có hiệu quả tương tự bổ sung sắt acid folic và đa vi chất trong việc duy trì tình trạng hemoglobin trong khi mang thai. Vì vậy RUSF Hebi-Mam sản xuất trong nước có thể trở thành một sản phẩm hứa hẹn để đưa vào chương trình can thiệp của phụ nữ có thai ở Việt Nam, góp phần cải thiện tình trạng dinh dưỡng của phụ nữ trong giai đoạn quan trọng của thời kỳ mang thai.

Objectives: The objective of this study was to test the effect of a daily ready-to- sesupplement foods (RUSF) fortified with 21 micronutrients providing 225 kcal (Hebi-Mam) on hemoglobin status of pregnant women in 10 communes, An Lao district, Hai Phong city.

Subjects and methods: A group of 398 pregnant women with gestational ages of 6 to 16 wk were enrolled in a randomized controlled trial, in which study women in 3 groups received at least 20 wk of either iron (58 mg) and folic acid (400 µg) supplementation or multiple micronutrient supplementation including 30 mg iron and 400 µg folic acid or Hebi-Mam including 30 mg iron and 400 µg folic acid. Hemoglobin was measured at baseline (6-16 gestational weeks) and at near term (36 gestational weeks) by cyanmethemoglobin method.

Results: The three interventions were equally effective in increasing hemoglobin concentrations, with a mean increase of 3.3 g/l in iron/folate group, 2.7g/l in multiple micronutrient supplementation group and 3.1 g/L in food supplement group (p<0.01). The prevalence of anemia was 26.1%, 24.8% and 24.5%, respectively. Factors found to be important predictors of hemoglobin improvement were baseline hemoglobin (p<0,001), gestational age at baseline (p<0.001), and ferritin < 15 mcg/l (p<0.01).

Conclusion and recommendation: The RUSF Hebi-Mam was as effective as the multiple micronutrient tablet and iron/folate tablet in maintaining hemoglobin status during pregnancy. Therefore, this locally produced RUSF Hebi-Mam could become a promising strategy to be included into maternal intervention programs in Vietnam, to contribute to the optimal nutrient intake and to improve nutritional status of women during the critical period of pregnancy.
